Fixes:

Fixed a crash when attempting to set effect parameters on a voice with no effect chain
Fix the default channel mask for 7.1 audio to match OS (and SDL) defaults
Wine/Win32: Use AUDCLNT_STREAMFLAGS_AUTOCONVERTPCM instead of manually matching output wave formats
