Changed

• The two-space left margin added in 2.11.0 is now opt-in via --margin,
  and disabled by default. Previously it was always on with no way to turn
  it off, even with --no-colour or --columns 0.

Fixed

• --margin now reduces the effective render width by 2 columns, so wrapped
  output plus the margin never exceeds --columns N. Previously the margin
  was added after wrapping, so the default 80-column setting could produce
  82-column lines.
