New languages

• Elixir (.ex, .exs): defmodule, def, defp, defmacro, defmacrop, defguard, defguardp, defprotocol, defimpl, defstruct, defexception, defdelegate
• Bash (.sh): function definitions (both greet() {} and function deploy {} syntax)

Changes

• Added call_entity_identifiers field to LanguageConfig for languages where entities are call nodes (like Elixir)
• 16 languages now supported: TypeScript, TSX, JavaScript, Python, Go, Rust, Java, C, C++, Ruby, C#, PHP, Fortran, Swift, Elixir, Bash
